You hereby agree to the general terms and conditions below when taking part in our refer a friend program

• The Friend you refer cannot be an existing patient previously known to Mind Body Teeth.

• Your FREE gift of our Sonic Toothbrush only applies once the person referred makes an appointment at Mind Body Teeth and attends their appointment.

• Your FREE gift of our Sonic Toothbrush is non transferable (cannot be transferred to another party) and cannot be exchanged for cash or any other form of payment.

• One FREE Sonic Toothbrush is gifted per patient that is referred (and attends their appointment). So if you make 3 successful referrals and each referred patient attends their appointment, you would be eligible for 3 FREE Sonic Toothbrushes.

• Our Sonic Toothbrush has a retail value of $AUD50.
