Refer a Friend Terms and Conditions

You hereby agree to the general terms and conditions below when taking part in our refer a friend program

The Friend you refer cannot be an existing patient previously known to Mind Body Teeth.
Your 10% discount only applies once the person referred makes an appointment at Mind Body Teeth and attends their appointment.
Your 10% discount is non transferable (cannot be transferred to another party) and cannot be exchanged for cash or any other form of payment.
Only one 10% discount can apply per appointment. So if you make 5 successful referrals, you would get a 10% discount on your next appointment, and be entitled to a 10% discount across 4 future appointments.
The total amount discounted per appointment per person cannot exceed $200. For example:

- If you or your friend's appointment fee is $280, you or your friend would receive a discount of $28.

- If you or your friend's appointment fee is $2000, you or your friend would receive a discount of $200.

- If you or your friend's appointment fee was $4000, you or your friend would receive a discount of $200.
